Returning home after residential treatment can be a time of hope, change, and uncertainty. It’s a crucial phase- one that requires the right support to build on progress and move forward with confidence. Designed for young people (16-25) who need more than weekly therapy but not full-time care, our Care Programme blends structured psychological support, mentoring, and practical guidance. It’s a personalised, hands-on approach that helps mentees regain confidence, strengthen resilience, and take meaningful steps toward independence.

Each mentee is supported by both a psychologist and a mentor - two dedicated professionals working together to provide stability, accountability, and encouragement. Before discharge, we collaborate with clinical teams to create a tailored transition plan, ensuring the right structure is in place from day one. Recognising that family dynamics play a key role in recovery and long-term success, we also offer family sessions to strengthen communication, address challenges, and ensure everyone feels informed and supported.
